Background
We've recently updated the Acceptable Ads criteria. However, a forum post referred to the bullet points on our application form to quote the criteria. Those points are outdated by now, however, and should therefore be removed.
Also note that the form is missing a title so while on it we could also add that one.
What to change
Add a <title> element with the text:

- No animation, it has to be static
- Preferably text-only

- This is an old version of the form, with outdated information and limited functionality since it doesn't allow the user to define their partner type - providing extra work for the whitelisting team to separate out the requests. Therefore I suggest removing this form.
- The user should be taken to this form instead: https://acceptableads.com/en/solutions/get-whitelisted, so we are not having to update 2 versions of the form.
- The form should open in a new tab.
- Also, update the text under Filling out an application on https://eyeo.com/en/services:
- From: You can do this by filling out this form <link>.
- To: Apply to get your ad whitelisted by filling out this form <link>.
- So when they open the link to the application form, the title makes sense.

Why are the points outdated? Those general criteria didn't change. The purpose of having them bullet pointed was to summarize the most important guidelines to make sure that the applicants went through them before applying. Having to click on a link to read them on a separate page (a link to the criteria is already in the form anyway) before applying isn't realistic unfortunately.
To summarize it all, those general guidelines are there to help the user know what our general criteria are. We can replace them by the update ones, but I don't think that they should be removed.
