ABP uninstall page: Add reinstall button to the data submission page

Description
Background
When a user uninstalls ABP from their browser, they are brought to an uninstall page with a list of options asking for reasons they have uninstalled ABP, and a button to reinstall ABP.
After submitting an answer, the reinstall button disappears with no other further action on the page provided. This is a lost opportunity to encourage users to reinstall the extension.
Since we introduced the reinstall button on the first uninstall page we have seen approximately 2000 additional reinstalls per day.
What to change
Keep the reinstall button on the submission page as well, and display the Data submitted successfully message below the button.

Saw the discussion in IRC, there you go fixed it for you. For reference check out the modules page if you're not sure which module an issue falls under. Once you know the module make sure to add the module owner and any peers as Cc, then they will be notified and know to triage the issue.
