After the host went down again; neither we nor Hetzner could find any issue. It's back in balancing for now, if it continues to be unstable I recommend decommissioning.

After analyzing the logs and performing some hardware-tests, another hard-drive has been flagged defect (the other one had just been replaced a few weeks ago). Hetzner has replaced the hard-drive, and currently the RAID is being re-synced.
